How care starts

Starting care does not need to feel complicated.

Whether you are arranging care for yourself, helping a parent, supporting a relative, planning a hospital discharge or thinking ahead, we keep the process straightforward and personal.

  1. 1

    Start with a conversation

Contact us and tell us what is happening and what support you are considering. You do not need to have everything worked out — we will listen and answer your initial questions.

  2. 2

    Getting to know you

If you would like to go ahead, we arrange an assessment. Family or other important people can be involved where you wish. It may consider:

    • Health and care needs
    • Routines and preferences
    • Abilities
    • Communication needs
    • Medication arrangements
    • Mobility
    • Interests
    • Goals for support
  3. 3

    Agreeing your care and support

Together we agree a personalised care plan setting out the support required, how it should be delivered, your preferences and important information for care staff. Before care begins we also discuss proposed arrangements, visit requirements and applicable charges.

  4. 4

    Building your support team

The relationship between you and your care staff matters. We consider your needs, preferences, the skills required and experience when identifying suitable team members. Where you wish and where practical — subject to staff suitability and availability — you may be involved in matching and possibly meeting members of your support team. We aim for consistency wherever reasonably possible.

  5. 5

    Your care begins

Once the assessment, care plan and arrangements are agreed, care can begin. Your care team will have the information they need to understand how you want to be supported.

  6. 6

    We keep listening

Care beginning is not the end of the conversation. We welcome feedback, review care and update arrangements when needed. If something is not working — timing, the support provided or compatibility with your support team — please tell us.

Our aim is to work with you so that your care continues to reflect what matters to you.

All care is subject to an individual assessment and Azan Home Care being satisfied that we can safely and appropriately meet the person’s needs.
