What may help

Support when you return home

Following assessment, support may include:

  • Personal care
  • Everyday routines
  • Dressing and grooming
  • Meal and drink preparation
  • Medication support, where appropriate
  • Mobility support in line with the care plan
  • Shopping
  • Essential household tasks
  • Companionship and reassurance
  • Support attending follow-up appointments
  • Re-establishing everyday routines
  • Monitoring and reporting relevant changes, as set out in the care plan
The exact support provided will depend upon the individual assessment and agreed care arrangements.

Short-term or ongoing care

Not everyone coming home from hospital needs permanent care. Some people need support for a while as they recover, regain confidence and rebuild independence; others find ongoing help useful. Arrangements can be reviewed as circumstances change.

Supporting independence

Our aim is not to take over the things someone can safely do for themselves. We encourage people to stay involved, keep their routines and make their own decisions, with support where it is needed.

Support for families

Hospital discharge can be stressful for relatives too — especially when family cannot provide all the support needed. Planned care can offer reassurance for everyone.

Planning ahead

Planning for discharge

Where possible, contacting us before discharge allows more time to understand circumstances, assess needs and consider the support required. Where appropriate, an assessment may be completed before care begins.

You do not need to wait until you are home to start the conversation.

If discharge has already taken place, you can still contact us to discuss the support you may need.
